IMPORTANT INFORMATION REGARDING APPE ENTRY TIME FRAME:

All students  can begin to submit choices for Advanced Pharmacy Practice Experiences(APPES).

Everyone will be asked to submit rank choices for fall/spring experiences at a later date. Choices submitted for the summer cannot be rolled into the fall and spring. Please note as well that not all faculty members, volunteer preceptors, and sites are available at all times, but your choices will be considered in the rank order that you submit.

Kindly enter your choices in EMS using the same procedures and guidelines previously given to you. The last day for you to enter preferences is March 30, 2010 at 11:59 PM. Due to the complexity of the system, there will be NO EXCEPTIONS to this process. All students must make sure that they have entered their choices NO LATER THAN March 30.  If you are in the middle of entering your choices at 12 midnight, your choices will not be saved automatically -- so do not wait for the last minute. Please remember to SAVE your selections for each course/advanced practice experience. If you do not submit choices, you will receive random selections from the EMS computer. A separate "spin" or lottery is conducted for each rotation in order to give everyone an equal chance of being lucky or unlucky!

All choices MUST be entered EXCEPT for those sites in which you have worked or are currently employed. Completing a prior college experience at a site (such as an IPPE – PH 311 or PH 312) is not a reason for not selecting the site. If you do not submit all choices for a given experience, your selections will be voided -- meaning, for example, if there are 70 electives you must rank all 70 and not your top 3 choices. In addition, as noted in the EMS directions which were given out a while and are available under the Forms and Documents heading in EMS as well.

For those of you who have been employed at any of the sites listed- the time frame of how long or when does not matter - you cannot enter a site that you have been previously employed at for any length of time. IF after the results come back it will be YOUR responsibility to email me ASAP regarding that failure to do so will result in you repeating that experience.

Again, you will have from March 13, at 12:00 AM to March 30 at 11:59 PM to enter your choices – DO NOT  forget to do this.

IMPORTANT APPE GROUP ASSIGNMENT:

Please note that the APPE GROUP Assignments will be available by tomorrow on EMS in the Forms and Documents section.

As we informed you at the APPE information session in February some of you may love the group that you are assigned to and some may not.

Please note that we will give EVERYONE a chance to switch Group Assignments  if they so choose HOWEVER the condition being

YOU find a person to switch with and you BOTH submit a signed request to switch your Group assignment number. Once approved you will be following the new schedule for the Group that you requested. We will NOT  be keeping a list to make switches it will be your responsibility to find another student who is willing to trade.

There is a strict time frame which will be enforced due to the constraints of the EMS system.

You will have 2 weeks beginning  from March 22- April 2 at 5pm to submit your Request. NO EXCEPTIONS !!!!!!!!!

IN addition you will find the new updated PharmD D Calendar for 2010-2011 in the same section please use this to guide you in your Preferences Entry note that once all spins are done and complete you will ONLY receive your schedule for the SUMMER at this time later on you will enter your choices again for the Fall/Spring.

EMAIL SENT FROM DEAN CICERO REGARDING NYS BOARD PART III EXAMINATION:

Special announcement regarding NYS Board Part III Examination

If you anticipate taking the Part III exam in June 2010 this announcement applies to you:

You are eligible to take PART III (compounding examination) ONLY if you have completed ALL fifth year courses by the end of the Spring 2010 semester AND have DOCUMENTED PROOF that you  have completed 1040 PHARMACY INTERNSHIP HOURS since securing your NYS pharmacy intern permit.

IF you meet the above requirements, then you must submit by April 1, 2010: It is strongly suggested that these forms be mailed in by March 15th to improve your chances of taking the test at LIU.

1.    Form 1 “to the Board Office” (Application for Licensure and First Registration) and a check or money order for the fee of $339.  This form  is available at:   www.op.nysed.gov Remember Form I must be signed and notarized

2.    Form 4 “to the Board Office”(Certification of Completion of an Internship In Pharmacy). DO NOT delay this form for payroll records.  Indicate hours worked as outlined, have this form signed and notarized and mail it with Form 1 (or separately if Form I has already been        mailed).

3.    Examination Application “to Castle Worldwide” with check or money order for the fee of $205 –application is available at www.castleworldwide.com

EMAIL SENT FROM MS. DURRANI REGARDING HIPAA:

As you may recall at the APPE information session held on February 3, you were informed that as soon as the online HIPAA access was available you would be notified via email.

Please note that as of today you can access the online HIPAA training modules at the following website( www.school.learnsomething.com)  - remember that instructions were given out in the packets that you received that day. For those of you who may have misplaced those instructions please go to the "Forms and Documents" section in EMS and reprint a copy of the instructions.

Once you have completed the training please printout the completion sheets and make several copies you will hand in one copy with your completed health form in April.

For any reason if you have trouble accessing the training modules please email me so that I can resolve the issues.

IMPORTANT INFORMATION FOR 6TH YEAR ROTATIONS:

HEALTH FORMS will be collected starting THURSDAY, APRIL 1ST and the last possible day to hand in the forms will be FRIDAY, APRIL 30TH.  There was a checklist handed out which you should follow to make sure you have all the following items before handing it in.  

If you don't remember your username and password to EMS please email Ms. Durrani as soon as possible.

Lastly, please check the website frequently and also your LIU emails for updates from the experimential office.

INFORMATION REGARDING SURVEYS FOR THE YEARBOOK:

PLEASE DOWNLOAD THE SURVEY BELOW AND ATTACH IT AND SEND IT TO liuyearbook2011@gmail.com, ASAP!

There are 3 lines for people whom you can choose- you do not have to choose 3 people, you may put 1-3 people (whatever you want to do). Once all of the surveys are collected, all the information will be gathered and than an online survey with the top 4-5 people will be posted for everyone to pick anonymously.
